• Birth certificates for all dependents under the age of 18
• Household income for the past 30 days (Work, Unemployment, Disability, Self Employment Affidavit, Child support, Gov. Assistance Program)
• Immunization Record (Current TB Test)
• Current Physical Form
• Proof of Residency
